Older Adults: Implementing Team-Based Health Care

Older Adults: Implementing Team-Based Care is a hybrid course, consisting of 8 online video modules, and one in-class session. This course is offered through University of California, San Diego Extension (UNEX). The on-line modules cover the following topics:
- Module 1 : Introduction: The Virtual Team and Care Coordination
- Module 2 : Teaming and Improved Coordination
- Module 3 : Supporting Patient Self-Management
- Module 4 : Supporting the Person as a Whole Person
- Module 5 : Gerontology Basics
- Module 6 : Working with the Disability Community
- Module 7 : Practical Guidance on Patient Protections
- Module 8 : Problem Solving Specifics for San Diego Patients and Providers
Students proceed sequentially through the modules, completing “Test Your Knowledge” questions before proceeding to the next module. After completing all of the modules, students from all disciplines convene at an in-class session
