Application Form
This program utilizes a joint online application developed by a consortium of California postbaccalaureate programs. The application allows you to apply to this program and simultaneously to other programs in the consortium, if you wish. Although the application delivery system has been centralized, your application will be considered separately by each program to which you apply. The application, including a list of participating programs, is located at http://meded-postbac.ucsd.edu

In addition to submitting a complete online application (you must fill out all required fields), you must submit supporting materials — letters of recommendation, college transcripts, and, if you are a reapplicant, a copy of your processed AMCAS application — to a centralized service, which will distribute copies of your materials to each of the programs that you designate on your online application. Mail supplemental materials to the address provided below.

The following documents will be required of "Accepted Applicants" prior to matriculation into the UCSD School of Medicine Postbaccalaureate Program.
- A copy of your 2008 Federal and State Income Tax returns or, if you are a dependent, your parents' current tax returns.
- A recent 2 x 2 photograph.
- Xerox copy of your health insurance card (if available).
