School Loans

• If you are a first-time Loans for Disadvantaged Students (LDS) Loan recipient :
  download and complete the Loans for Disadvantaged Students (LDS) Loan Promissory Note.


• If you are a first-time Perkins Loan recipient :
  download and complete the Perkins Loan Master Promissory Note.


• If you are a first-time University Loan recipient :
  download and complete the University Loan Master Promissory Note.


• If you are a first-time California Medical Association Foundation (CMAF) Loan recipient :
  download and complete the CMAF Promissory Note.


• All school loan recipients must download, complete and submit the Information Sheet along with the appropriate school loan promissory note(s).


• Loan Disclosure Statements for students receiving Perkins, University, Loans for Disadvantaged Students, CMAF and/or AMA Loan Awards . Click on "Health Sciences student loan disclosure" under "Tools".
