To Receive Transcripts:
- Fee is $6 per copy.
- Make check payable to "UC Regents".
- Include current address, phone number, or e-mail address where the alumnus may be contacted in case questions arise.
- Include address where transcript is to be sent.
- If additional undergraduate or graduate education was received at UCSD, please indicate if those academic records should be included. There is no extra charge.
To Receive Dean's Letters:
Free of charge.
If a large number of dean's letters needs to be provided, please supply address labels. Otherwise, please send addresses, along with alumnus written consent to send the dean's letters.
Include current address, phone number, or e-mail address where the alumnus may be contacted in case questions arise.
To Receive Medical degree/medical education verification:
Free of charge unless a transcript is needed.
When sending any forms for medical degree/medical education verification, please indicate where the form should be mailed upon completion.
