Drug Enforcement Administration (DEA) Registration

UCSD offers a fee exemption for your DEA registration. However, please note that a DEA registered with the UCSD fee exemption is only valid within the scope of your training. This applies to rotations at affiliate hospitals, but will not be valid while moonlighting.

 

Initial Registration

You may register with the DEA on-line at http://www.deadiversion.usdoj.gov/ External Site / New Window. As a housestaff member of the UCSD Medical Center, you qualify for fee exemption. To receive this exemption, you must enter the following information:

Note: You MUST check "CERTIFICATION FOR FEE EXEMPTION" in order to receive the fee exemption.

When you receive your DEA number, please provide a copy to the Office of Graduate Medical Education, Mail Code 8829 and to your training program coordinator.

 

Renewal

To renew your DEA Registration online, please visit the DEA Website External Site / New Window.

 

 

Controlled Substance Prescription Pads

After receiving your California medical license and DEA number, you will need to order Controlled Substance Prescription Pads at the UCSD Medical Center. Please send the Controlled Substance Security Prescription Pad Order Form Static Form: Fill out and Print along with a copy of your DEA registration certificate to:

Controlled Substance Security Prescription pads will be mailed back to the address on your DEA certificate.

To re-order Security Prescriptions, please use the on-line ordering system External Site / New Window. You will be prompted for your Username - Your prescription Lot No. located in the lower left of your prescriptions, and your Password - Your California Medical License number.
