Applicant's California medical licensure or eligibility to train while unlicensed
Proof of professional liability insurance from applicant's full-time training program. [Any waiver of this requirement must be approved, in writing, by the UCSDMC Department Chair]
The Program Director will write a letter of recommendation to GME to request appointment. The letter will note:
Applicant's present position
Proposed activity within the UCSD department
Appointment date
Licensure status in this state
The Program Director will provide the following:
Letter of good standing signed by the training program director in the applicant's full-time GME training program
Rotation Schedule form - [47 kb], completed [Please also enter in block scheduling once they are entered in New Innovations]
Proof of current professional liability coverage
Medical school diploma
Applicant's training program's current ACGME accreditation letter
The Program Coordinator will process ALL applicable employment forms with the new appointees before the new trainee's appointment start date.
The Clinical Department will enter the appointment data into the Payroll Personnel System (PPS).
Data on new hires may not be entered more than 60 days prior to the effective date of the action.
If the applicant will be scheduled for rotation to the VA Medical Center during their elective residency, the clinical department will:
Annotate its current VA TQCVL letter and roster to add the elective resident
Email the revised TQCVL and roster to Sandra Bee and Cindy Slaughter . Please note in the email that the attached TQCVL has been updated to include the elective resident.
Prepare an original of the TQCVL on department letterhead and obtain the Department Chair's signature on the document. Please send the original chair-signed paper copy to Sandra Bee, mail code 9142. Faxed documents will not be accepted.
As an elective resident, the applicant will:
Be appointed as a nonsalaried Resident Physician 1 through 9
Not be eligible for participation in the housestaff group health insurance program
Provide his/her own uniforms
When the above steps are completed, Graduate Medical Education will do the following:
Sign in the appointee. Please have the appointee follow these procedures so that the trainee is properly registered.
Provide the trainee with a physician ID number and access to the Medical Center's computer system.
Register the trainee, as needed, with the Medical Board of California
