download and complete the Loans for Disadvantaged Students (LDS) Loan Promissory Note

If you are a first-time Perkins Loan recipient :
download and complete the Perkins Loan Master Promissory Note. -
If you are a first-time University Loan recipient :
download and complete the University Loan Master Promissory Note. -
If you are a first-time California Medical Association Foundation (CMAF) Loan recipient :
download and complete the CMAF Promissory Note. -
All school loan recipients must download, complete and submit the Information Sheet along with the appropriate school loan promissory note(s).
