The ultrasound (labeled D) shows the classic appearance of a single large gallstone within the gallbladder. An intraluminal reflection as well as marked acoustic shadowing behind the stone is present. The transverse scan (E2) also demonstrates a single reflecting density and, therefore, one large shadow posterior to the gallbladder. The shadowing obscures the detail of the right kidney.

Ultrasound is usually the preferred radiographic study for confirming acute cholecystitis. The study is both sensitive and specific and can be done rather quickly. Typically the ultrasound is the only study needed in a classical presentation of acute cholecystitis.
