ASSESSMENT
  1. Comment on the mode of breathing patient is on.
    Room air?
    Face mask/Nasal canula?
    Mechanical ventilator?
  2. Ventilator settings:
    Mode, FiO2, rate, tidal volume etc.
    PEEP-2-10 cm H2O?
  3. Oxygen saturation
  4. Peak airway pressure
    Overbreathing ventilator (pt rate>>> machine)
    Fighting ventilator?
  5. ABG:
         Pa02 >60 mm H20
         Pa C02- 40 mm H20
         PH- 7.35-7.45
  6. Fluid status: Pulmonary edema can develop
    Net positive?- fluid overload.
    Net negative?
    Diuresing? - Can relieve pulmonary edema.
  7. Physical Exam: Listen to patients lungs daily.
    Fighting ventilator? Comatose?
    Secretions?
    Lungs: clear. Rales? Ronchi? Wheezing?
  8. Imaging: Chest xray: ET tube in right place, 3-5 cm above carina.
    ARDS, pneumothorax, pleural effusion, pneumonia.

Problems:

  • Pulmonary hygiene
  • Intubation and mechanical ventilation needed
  • ARDS
  • Pneumothorax
  • Ventilator Problems
