Forms to submit with ISP Proposal:

• Form C — Independent Study Project Proposal Form (PDF - 48 kb | Word - 39 kb)

   

• Form D — Human / Animal Subjects Investigation Form (PDF - 86 kb | Word - 36 kb)

  IMPORTANT!

  All students must submit this form even if human or animal subjects are not used.

Forms to submit with completed ISP:

• Form 1 — ISP Completion Verification/Chair Evaluation  (Chair only) (PDF - 35 kb | Word - 37 kb)

   

• Form 2 — ISP Committee Member Evaluation  (one for each non-chair Committee Member) (PDF - 32 kb | Word - 34 kb)

   

• Student evaluation of ISP experience via CourseEval

• ISP Library Consent - optional (Currently the UCSD Biomedical Library stores hard copies of all ISPs.  This consent permits the library to move to electronic storage of ISPs on a password-protected site.) (PDF - 56 kb | Word - 30 kb)

• Note: The final ISP write-up should be submitted via email to Catherine Doran.
