Requirements:

Calendar:

Fall, Winter and Spring quarters; program for 2009/10 begins 9/21/09.
Students must adhere to UCSD's schedule.

Deadline:

Three months prior to clerkship.

Procedure:

To receive an application, please submit the following via fax (no earlier than 8/1/2009):

• Letter of interest which includes:

1. name
2. mailing address
3. medical school / location
4. date of receiving M.D. degree.

• List of clinical experience which includes: name / location / dates of clinical work. [Surgery applicants: submit clinical evaluation (description) of SURGERY clerkship, minimum 4 weeks, taken in country where English is the native tongue.]
• Submit evaluation in speciality of your elective choice (except Radiology). 
  Evaluation must describe in detail your clinical performance, written and signed by physician/faculty who worked directly with you.
• Provide health record, current immunizations including TB test within one year of elective dates.
• Proof of Personal Health and Accident Insurance.
  
• Proof of Medical Malpractice Insurance.
• Provide Dean's Letter.
• Submit material written in English ONLY.
  
  

  After eligibility is determined, official application materials will be forwarded.
